The extra sound may precede the normal first heart sound (When it is referred to an atrial or 4th heart sound) or follow the normal second sound (when it is referred to as a ventricular filling or 3rd heart sound. The extra sound when pathological in origin is separated from the normal sound by a readily appreciable gap and has a different quality, being dull or muffled in nature. The tachycardia which often accompanies cardiac failure makes precise timing difficult or impossible and under these circumstances a characteristic cadence can often be heard to which the descriptive term gallop rhythm has been applied.
