The extra sound may precede the normal first heart sound (When it is referred to an atrial or 4^th heart sound) or follow the normal second sound (when it is referred to as a ventricular filling or 3^rd heart sound. The extra sound when pathological in origin is separated from the normal sound by a readily appreciable gap and has a different quality, being dull or muffled in nature. The tachycardia which often accompanies cardiac failure makes precise timing difficult or impossible and under these circumstances a characteristic cadence can often be heard to which the descriptive term gallop rhythm has been applied.
