Diagnosis. Acute pyelonephritis should be distinguished from:

    1. Acute appendicitis, salpingitis and diverticulitis, especially by the absence of pus and organisms in the urinr.
    2. Diaphragmatic pleurisy with or without pneumonia. Pain with guarding similar to that occurring in pyelonephritis may be present, but there are no abnormalities in the urine and abnormal physical sings may be detected in the chest.
